Document GitHub Enterprise Apps and existing GitHub App setup - #2114

Add a "Using a GitHub Enterprise App" section for installing one private App across the organizations in a GitHub Enterprise Cloud enterprise. Sourcegraph's Create GitHub App flow can't register an App owned by an enterprise, so the App is created on GitHub and added to Sourcegraph. Expand "Adding an existing GitHub App" into a full procedure: generate the private key and client secret, add the App, install it, add the auth provider, and create a code host connection. Add webhook setup, since Sourcegraph doesn't create an incoming webhook for an existing App. Add a shared "GitHub App settings" table with the values Sourcegraph uses when it creates an App (URLs, permissions, install scope), and link to it from the create flow instead of repeating the permissions list.
