A piped password caused the dashboard in 1.11.0 to reject an otherwise interactive Unix terminal. Accept Crossterm’s /dev/tty input fallback while retaining the terminal requirement for stderr. Remove the duplicate “An error occurred” prefix when dashboard startup fails.

Also improve CLI help with command examples, clearer account and vault terminology, input formats, index numbering, and explicit descriptions of unencrypted exports. Clarify password prompts, save and deletion messages, and dashboard shortcuts.

Fixes #695

Validation:

  • cargo test --locked: 78 passed, 1 ignored.
  • cargo fmt --all -- --check and cargo clippy -- -D warnings passed.
  • Manually verified normal and piped-password launches in a PTY: the dashboard renders and q exits successfully.
  • Checked help output for every subcommand; added a regression test asserting a single error prefix when terminal output is unavailable.
